About Us

Bridging Care was created as a joint venture between two 501(c)3 nonprofit organizations: Washington Academy of International Medical Graduates (WAIMG) and Angels for Angels. The joint venture's goal was to create an environment where diverse patients could direct their healthcare. We utilize our team members and include partnerships that strengthen the fabric of what we stand for as an organization: a world where health equity is celebrated through whole patient care. It is through these partnerships that we strive to empower the community we serve. We track and define our success through outcomes that lift barriers, create access, and address community-identified needs.

Current Opening

Bridging Care, a WA State entity, seeks a full-time Care Coordinator.

Travel: 50% time; Travel to patient sites in Snohomish, King and Pierce County required.

Description:

  • Client Engagement & Advocacy: Build and maintain strong, trust-based relationships with clients, ensuring their active involvement in all aspects of their care journey and promoting autonomy in healthcare decisions.

  • Cross-Functional Collaboration: Partner with multidisciplinary teams, including healthcare providers, social services, and community organizations, to create cohesive, individualized care plans that address complex health needs.

  • Resource Navigation & System Optimization: Identify, access, and utilize community resources to remove barriers to care, improve patient outcomes, and enhance the quality of life for medically complex individuals.

  • Comprehensive Health Assessments: Conduct thorough, holistic health screenings covering physical, behavioral, and social determinants of health to inform personalized care strategies.

  • Care Coordination & Case Management: Lead the coordination of care across multiple touchpoints, ensuring seamless communication among all stakeholders and timely delivery of services.

  • Health Promotion & Education: Develop and implement targeted health promotion strategies, empowering clients to take proactive steps in managing their health and preventing hospital readmissions.

  • Data-Driven Decision Making: Utilize electronic health records and case documentation systems to track patient progress, measure outcomes, and inform continuous improvement efforts.

  • Cultural Sensitivity & Competence: Leverage bilingual communication skills and cultural competence to effectively serve diverse populations, ensuring equity and inclusivity in care delivery.

Requirements:

  • Bachelor of Applied Science (B.S) in Public Health or related

  • 5 years experience in healthcare provision/medical setting

  • Certification and Trainings in following core areas:

    • Nursing Assistant

    • Bloodborne Pathogens

    • Adult and Pediatric First Aid/CPR/AED

    • Adult and Pediatric First Aid/CPR/AED-BL

    • HIPAA Security and Privacy Training

  • Language Fluency in one or more languages (other than English)
