What exactly does a Care Coordinators do?
Care Coordinators helps coordinate all aspects of a senior’s health and daily support so nothing falls through the cracks. We stay connected with medical providers, monitor needs, help solve problems as they arise, and keep families informed. Our role is to make aging at home safer, easier, and more supported.
How is this different from a home care agency or caregiver?
Home Care Agencies provide caregivers who help with daily tasks in the home.
Bridging Care provides a Care Coordinators who oversees the whole picture — medical, emotional, logistical, and family support — ensuring that your loved one can stay safe and independent at home with the right services in place
How is this different from what my doctor or primary care provide already does?
Doctors treat medical conditions but they can’t manage daily logistics, follow-up appointments, medication coordination, insurance confusion, care transitions, or home safety risks. We act as the bridge between medical care, home support, and family needs — helping everything work together smoothly.
Who uses this service?
Some seniors use our support to stay independent and active.
Other times, adult children use us as a trusted partner to reduce stress, stay informed, and make sure their loved one has someone watching out for them. In many cases, we support the whole family, not just the person aging.
How much do the plans cost?
We offer three levels of support depending on how much communication, coordination, and hands-on involvement is needed. Each plan includes a dedicated Care Coordinators, monthly check-ins, ongoing support, and coordination services.

Do you accept insurance, Medicare, or Medicaid?
At this time, our Aging-in-Place Senior Care Plans are private pay only.
Insurance rarely covers this type of proactive coordination, planning, and advocacy — even though families consistently tell us it is the most valuable support they receive. However, we can often help families maximize insurance-covered services and avoid costly gaps in care.
Can we change plans later if our needs change?
Yes. Needs can change — sometimes gradually, sometimes overnight.
You can move up or down between plans at any time. We’ll help guide you based on changes in independence, health, support needs, and family involvement.
What areas do you serve?
We currently serve seniors and families throughout Washington State, including: Seattle, Bellevue, Redmond, Kirkland, Renton, Tacoma, Everett, Olympia and surrounding communities across King, Pierce, Snohomish, and Thurston Counties.
We also provide services virtually statewide, so even if your loved one lives outside our primary in-person region, you can still receive support, coordination, guidance, and family communication through remote care management.
