Bridging Gaps to Provide Whole person Health

Working together to provide you or your loved one with a range of services to meet your everyday needs and bridge the gaps in your health journey.

Care Coordination

Our Bridging Care Therapist offers weekly outpatient behavioral health counseling through tele-health. Designed for individuals who need mental health, substance use disorder (SUD), or co-occurring support.

Behavioral Health

Our Bridging Care team will work with your primary care and the community to prevent gaps in your care. Whether you need support making appointments, finding assistance with food, transportation, housing, etc., we are here for you.

Who is Eligible?

  • The Health Home program is eligible for Medicaid-eligible individuals (health home). Contact us to check to see if you are eligible for free care coordination services with Bridging Care.

    Not eligible? We also provide a private pay option that is open to all.

    • Current BC Patients
      Once we are contracted with all Managed Care Organizations (MCOs), all existing BC patients will be eligible to receive counseling services.

    • Individuals Covered by Apple Health (Medicaid)
      We will be accepting clients enrolled in Apple Health through MCOs that we are contracted with. Please check back soon for a list of participating MCOs.

    • Private Pay Clients
      Counseling is also available for individuals who wish to pay out-of-pocket. We offer flexible options for those not using insurance.
